The second annual tournament will be supporting a Tampa Bay Area First Tee facility by providing a sun shade to protect the children while they practice their golf swings or Wear Your Awareness™ kits. These kits will include, yet are not limited to, a draw string backpack, UV shirt, UV hat, UV detector bracelet, sunscreen and specially designed age appropriate melanoma literature and activity book. A portion of the proceeds will also be donated to the Dean Hedstrom Foundation Research Fund at the University of California San Francisco.
Registration
Professional golfers and amateur golfers are welcome to registrer ONLINE or by filling out a registration form. By registering online, you will be able to view and print a summary of your registration as well as your tax-deductible receipt for your charitable donation to the Dean Hedstrom Foundation for Melanoma Awareness, a 501(c)3 NON-PROFIT organization. Detailed Tournament Information
Format
Five-person team scramble. Teams will consist of four amateurs and one professional. Professionals will play their own ball and amateurs will play a scramble format. The low score on the hole will count as the team score. Professionals will compete for a separate Professional Prize Purse.
Teams
60 team maximum (4 amateurs + 1 professional per team). Individual players and/or teams with less than four players will be paired with other players to make a complete foursome. Professionals can bring their own team, or will be paired with a team on the day of the tournament.
Entry Fee
Amateurs: $1000 per 4-person team or $250 per player
Professionals: No entry fee. In lieu of entry fee, donation is requested for round of golf for four at your facility or facility of your choice.
Entry fee includes: Greens fee, cart, use of driving range, lunch, dinner, team listing in program, gift bag, and invitation to dinner and auction.

No Show/Withdrawl
Any player who no-shows for the event will forfeit their entry fee. A player may withdraw without penalty up until 6:00 pm Friday, April 23, 2010, by contacting Pat Walsh, Tournament Director at pat@dhfma.org. A player withdrawing after the deadline but before 6:00 pm Friday, April 30, 2010 will be charged $200.00. Players who withdraw after 6:00 pm Friday, April 30, 2010 will be considered a No-Show and will not be refunded for any reason. A life or death circumstance will be the only situation that could alter this policy.
Suspension of Play (Due to Dangerous Situation)
Play may be suspended due to dangerous situations. After a horn or siren has sounded, players must immediately halt play and may not resume again until the Committee has ordered to do so. Under suspension, the entire golf course, driving range, putting greens and all practice areas are closed. Violation of this rule will result in disqualification.
Rainout Policy
Due to unforeseen inclement weather, the occasional "rain-out" is inevitable. Please make early telephone contact with the Golf Course Pro Shop at (727)442-0229 should there be stormy or uncooperative weather on the day of the event. Should the course be deemed unplayable when less than nine of 18 holes have been completed, the golf course will issue each player a rain check for 18 holes of golf. Nine holes or more completed shall constitute a complete tournament and scoring will be based on the team's first nine holes in relation to par.
The Dean Hedstrom Foundation for Melanoma Awareness, Inc reserves the right to refuse entry to any player for any reason.
